special service, simply complete and
submit the form on this page or download
and email or mail a word version of the
form to us.

Remember to include dates, times, your
church & pastor's name, a contact name
and phone# or email address. Describe
the event or service, giving as much detail
as possible and whether you are
requesting the church, a department or
auxiliary.

Please follow up with a letter from your
Pastor or Church leader addressed to:
